
We are delighted to share that Dr Soong Jie Lin, Lecturer (Practice Track), has been awarded the Gold Award for Best Oral Presentation at the Asian Conference of Clinical Pharmacy (ACCP) 2026 in Bangkok.
The award recognises the study, “Evaluation of a Medication Reconciliation Risk Stratification Tool (MR-RST): A Two-Phase Study in Singapore General Hospital,” which highlights the importance of directing medication reconciliation efforts to the right patients at the right time to optimise patient care and resource utilisation.
This achievement reflects the dedication and collaborative efforts of a multidisciplinary team, including pharmacy students from the National University of Singapore and pharmacy colleagues, whose hard work and commitment over the past two years made this success possible.
Dr Soong shared “The future is not doing more medication reconciliation… but it’s about doing medication reconciliation for the right patient, at the right time, with the right intensity.”
